The Photocopy Log must be filled out with detailed information from each material you plan to include in your course pack. Please email a digital copy or attach a print copy of the log to your course pack order when submitting it for copyright approval.
The bookstore also requires you to fill out a request for printing form.
Please fill out the attached form and submit with all print requests sent to duplicating.
Library items and/or personal items can be placed on reserve in the library. The items are held behind the circulation desk and can be loaned out for 1, 2, 4, 24, and 72 hour increments at the instructors discretion.
When placing material on reserve, please fill out the reserve request form and hand in to the circulation desk along with your reserve materials.
If you wish to use material that is not provided for under Fair Dealing or other exceptions in the Copyright Act submit this form to the Copyright Office. Please submit this form as soon as possible to allow the copyright office time to obtain the appropriate permissions.
If you wish to use a students coursework as an example for future classes or other non-profit educational purposes, please fill out this form and provide a copy to the Copyright Office.
Note: The “Use of a Student Work” form is not be used if the work contains any personal image or information other than their name (e.g. Photographs of themselves or others).
If a personal image or other personal information is contained in a student work You must complete the “Consent for use of Personal Image and Information” form located on the FOIP Website.
Instructors are encouraged to keep records of all third party materials used in class for up to six years. Although audits by the Copyright Board are incredibly rare, it does fall within their mandate.
